New Patient Packets
Austin Heart provides our new patients with a New Patient Packet to help introduce you to the practice and streamline your first visit. As long as your appointment is three or four days from the day you call to schedule, we mail the packet to your home. This allows you to fill out paperwork prior to arriving to our office. The New Patient Packet includes:

Forms Needed for Your First Visit
List of Medications
We also ask you to be sure to bring a complete list of
all prescription and non-prescription medications that
you are currently taking. Write down the prescription
name, dosage amount, and how often the medication
is to be taken. It is very important that our physicians
receive accurate medication information.
